| Requirements and Responsibilities For Resident Care Coordinator |
| Coordinates, supervises, and evaluates the care of residents in a skilled nursing, assisted living, or similar facility. Schedules and trains care staff, and ensures that clinical services are delivered in accordance with regulations and professional standards. Communicates with families and medical professionals as needed, and ensures care plans for residents are properly documented and updated. Requires a high school diploma. May require Certified Nursing Assistant (CNA). Typically reports to a manager or head of a unit/department. Supervises a small group of para-professional staff in an organization characterized by highly transactional or repetitive processes. Contributes to the development of processes and procedures. Typically requires 3 years experience in the related area as an individual contributor. Thorough knowledge of functional area under supervision. |
